In 2021, CMS directed its MACs to remove the 90-day global for the two CPT Codes 28820 and 28825. Having said this, not all private insurance has gone along with the change that happened on 1/1/2021. It is found in the Federal register here https://www.federalregister.gov/documents/2020/12/28/2020-26815/medicare-program-cy-2021-payment-policies-under-the-physician-fee-schedule-and-other-changes-to-part
It is incumbent upon the doctor/billing staff to inquire with any private insurance and this includes Medicare Advantage as to what these two CPT codes are designated when it comes to their global periods.
