Jeffrey: it is important to start with the CPT code you will bill and that is CPT 11750. It is defined as "Excision of nail and nail matrix, partial or complete, (eg, ingrown or deformed nail) for permanent removal". This clearly means, any part ultimately to the whole nail. So, 2 borders are two portions and does not even equal a whole nail so it still meets the criteria of ONE UNIT of CPT 11750. On one toe, it billed as ONE UNIT maximum, period.
