While Medicare has classified toe amputations (CPT 28820 & 28825) with a "0" day global why does this not apply to all insurances?

Several years ago Medicare made these two CPT codes 28820 & 28825, ZERO day global. This means any subsequent visits are NOT post operative visits but they are evaluation and management (E &M) visits. This is often overlooked for the Medicare patients by providers. Now, private insurance may not follow Medicare rules and may keep these set at 90 days global. Next, toe amputation you do, please check first with that payer to see if they kept the 90 day global period or changed it to "0" day global as this impacts your reimbursement and documentation.
