There is a code for surgical shoes, L3260. Does Medicare cover these and if so under what circumtances?

If there is a request to submit to Medicare because the secondary covers L3260, then always append GY-meaning statutorily not covered and a RT or LT. Otherwise it is just a cash expense by the patient as Dr. Kesselman stated.
Surgical shoes, such as L3260 are considered dressing covers and are statutorily non covered under Medicare. Since they are not covered, no ABN is required and you can charge the patient cash.
